Weeks 1–2
Pain
Calm the pain down.
Hands-on work, functional dry needling and positions that unload the disc settle the irritated nerve and the muscles clamped around it, starting visit one.
What We Treat
You tie your shoes sitting down now. The first ten minutes out of bed are a negotiation. Sharp in the back, burning down one leg, or both: it has a cause, and one visit is enough to find it.

Why It Happens
Your back doesn’t hurt for no reason, and it’s rarely one thing. A disc can lose height and crowd the nerve root that feeds your leg, which is what sends burning past the knee into the calf or the foot. The small joints behind it stiffen and stop sharing the load. Your glutes and deep abdominal muscles switch off after a flare-up, so the same two or three segments absorb everything you do. And the way you sit, bend and lift keeps reloading that sore spot before it ever gets a chance to settle.
Sciatica is a symptom on that list, not a diagnosis of its own. That matters, because the fix is different depending on which item is driving it, and none of them resolve with rest alone. That’s why the third round of ice and a printed stretch sheet ends the same way the first two did. An evaluation that looks for the cause tests nerve tension, hip motion and segment by segment mobility, and screens your strength side to side. Then you know whether you’re treating a disc, a joint, a muscle, or the habit that keeps feeding all three.
The Honest Truth
Nobody regrets starting too early.
How We Fix It
Every plan has an endpoint. Here’s what the four phases look like for back pain and sciatica.
Weeks 1–2
Pain
Hands-on work, functional dry needling and positions that unload the disc settle the irritated nerve and the muscles clamped around it, starting visit one.
Weeks 2–6
Prime
Stiff segments and locked-up hips get their motion back, and nerve glides give the sciatic nerve room to move again.
Weeks 6–10
Perform
Progressive core, glute and hip strengthening builds the support your spine has been going without.
Weeks 10+
Prevent
Lifting, bending and twisting get tested under real load before you’re cleared to go all-in. That’s the endpoint, not just another appointment.
Questions, Answered
Usually not, and it’s rarely the first step. Imaging finds disc bulges and wear and tear in people who have no pain at all, so a picture doesn’t always point at the cause of yours. What it can’t show is which segment stopped moving, which nerve is irritated, or which muscle quit. Those come out of a hands-on evaluation, and they are the things treatment actually changes. We screen for the findings that belong with a physician, and if you need imaging, we help you get it.
Yes, and movement usually helps more than rest. Hurt doesn’t always mean harm. Total rest leaves a back stiffer and weaker, which is what sets up the next flare-up. The part that matters is dosing: the right movements, in the right direction, at a load your back can handle right now. That gets sorted at your evaluation and progressed one-on-one with a licensed physical therapist, who is watching you do it instead of handing you a sheet.
You’ll get that answer at the evaluation, once someone has actually tested your back. What we can tell you now is the shape of it: four phases, Pain, Prime, Perform, Prevent, each with a job and a way to know you’ve earned the next one. That’s a plan with an endpoint, not an open tab of appointments. We go over what it takes and what it costs before you commit, and a Good Faith Estimate is available if you’re paying out of pocket.
No referral needed. Louisiana is a direct access state, so you can be evaluated by a licensed physical therapist without going through a physician first. We run a benefits check before your first visit, so you know your coverage in advance. We accept Medicare, Medicaid, Blue Cross Blue Shield, Aetna, Cigna, Humana, UnitedHealthcare, TRICARE, TriWest and Peoples Health, plus workers’ compensation and auto accident claims.
We won’t promise you’ll avoid one, and anybody who does is guessing. Physical therapy is the conservative step that comes first, and it addresses the stiff joint, the crowded nerve and the missing strength that no procedure fixes for you. Plenty of back and leg pain settles right there. If yours doesn’t, you walk into that conversation knowing what you tried and what changed, which is a better place to decide from. And a strong back going in has a head start on the rehab that comes after.
