Weeks 1–2
Pain
Calm the pain down.
Hands-on work on the upper neck joints and the muscles under your skull, with functional dry needling where it fits, breaks the pattern. Starting visit one.
What We Treat
You take something at the first sign, before it owns the whole afternoon. The tight band at the base of your skull by 2pm. A lot of head pain starts in the neck, and that is treatable. It starts with an evaluation that looks for the cause, not another guess.

Why It Happens
A lot of head pain is referred pain. The top joints of your neck share nerve pathways with the nerves that carry sensation to your face and scalp. When those joints stiffen and the muscles under the base of your skull stay locked down, your brain reads that irritation as an ache behind the eye or over the temple. Hold your head forward of your shoulders at a screen or a windshield for hours and it compounds: the deep muscles that hold your head up fatigue, and the upper traps and the muscles under your skull take the load instead.
That is also why it keeps coming back. Medication does its work on the pain and none of it on the cause, so the pattern resets and waits. Stress adds jaw clenching and shallow breathing on top of tissue that is already irritated. It takes an evaluation that looks for the cause to sort out which one is driving yours: restricted joints in the upper neck, muscle tension and trigger points, how you load your head all day, or a mix. Name the driver and the plan stops being guesswork.
The Honest Truth
Nobody regrets starting too early.
How We Fix It
Every plan has an endpoint. Here’s what the four phases look like for headaches.
Weeks 1–2
Pain
Hands-on work on the upper neck joints and the muscles under your skull, with functional dry needling where it fits, breaks the pattern. Starting visit one.
Weeks 2–6
Prime
Rotation and side bending come back in the stiff upper neck segments, and the deep muscles in front relearn how to hold your head up.
Weeks 6–10
Perform
Endurance work for the neck, shoulder blades and upper back, so a long day at a screen or a wheel stops setting the ache off.
Weeks 10+
Prevent
Your desk, your sleep position and a reset you can do yourself get set up, so the pattern has nowhere to rebuild. That’s the endpoint, not just another appointment.
Questions, Answered
Usually not, and it’s rarely the first step. Imaging can’t show a stiff joint or a muscle holding tension, and those are the drivers we can actually treat. Scans also turn up wear and tear in people with no pain at all, so a picture doesn’t always point at the cause. We start with a hands-on evaluation, and we screen for the findings that belong with a physician instead. If you do need imaging, we’ll help you get it. You don’t have to wait on a scan to start feeling better.
Not when it’s dosed right, and dosing it is the job. Hurt doesn’t always mean harm, and complete rest tends to leave a neck stiffer and weaker, which sets up the next round. Your evaluation sorts out which movements settle your symptoms and which ones feed them. Then you get those, at a load you can handle, one-on-one with a licensed physical therapist. If something doesn’t feel right, it gets changed on the spot.
You’ll know the shape of your plan after the evaluation, not before it. Care runs through four phases: Pain, Prime, Perform, Prevent. Each phase has a job and a way to know you’ve earned the next one, so you always know where you are and what’s left. That’s a plan with an endpoint instead of an open string of appointments. We’ll tell you what it takes and what it costs before you commit to any of it.
No referral needed. Louisiana is a direct access state, so you can be evaluated by a licensed physical therapist without going through a physician first. We run a benefits check before your first visit, so you know your coverage in advance, and a Good Faith Estimate is available if you’re paying out of pocket. We accept Medicare, Medicaid, Blue Cross Blue Shield, Aetna, Cigna, Humana, UnitedHealthcare, TRICARE, TriWest and Peoples Health, plus workers’ compensation and auto accident claims.
Then we treat the part that belongs to us. Physical therapy doesn’t replace your physician or your medication, and we won’t pretend otherwise. What we can address is the neck stiffness and muscle tension that often ride along with a migraine and make it easier to set off. Plenty of people have both. Your evaluation tells us how much of your pattern is coming from your neck, and we’ll be straight with you if the answer is not much.
