Hands-on work opens a window. This is what goes in it. Loading the right tissue, in the right order, is the reason relief is still there a month after your last visit.
What It Is
Therapeutic exercise is not a workout, and it is not a printout. It is a specific set of movements, at a specific load, in a specific order, chosen because your evaluation showed which tissue quit doing its job and which one has been covering for it.
Here is the mechanism. Pain makes you guard. Guarding makes a muscle go quiet and lose capacity. Weak tissue hands its work to the joint next door, and given six more months, that joint starts complaining too. Hands-on work can calm the pain and give the motion back. Only load teaches the tissue to hold that motion under real life: a toolbox, a car seat, a kid on your hip, a full shift.
So this is the part of your plan with the endpoint attached. It carries you through the four phases, Pain, Prime, Perform, Prevent, and you move up by hitting something we can both see, not by finishing a number of weeks.

What A Session Looks Like
1
We check what changed since last time.
2
You work, with hands-on cues every rep.
3
We set the dose you take home.
What Changes
Lifting without doing the mental math first.
A full day of work, then an evening that’s yours.
Flare-ups that fade instead of moving in.
Progress you can point at, not just feel.

Questions, Answered
Move at home, please. What a video cannot do is watch you. People compensate without knowing it, so the muscle we are trying to reach never gets the work and the one already doing too much gets handed more. A licensed physical therapist watches the rep, changes it, and then sends you home with the version that actually loads the right thing.
Usually not. New load tends to produce a dull, spread-out ache that shows up hours later and settles down on its own. Sharp pain, pain inside a joint you were not working, or soreness that is worse on the third day is different information, and we want to hear about it. Tell us and we change the dose. Nothing here asks you to push through pain.
A gym program is built to make you generally stronger. This is built out of an exam of one problem: which tissue lost capacity, which joint has been taking the overflow, and what has to come back first. It also has phases. Pain, Prime, Perform, Prevent, in that order, each with a job and a way to know you have earned the next one.
Plenty of people here do, and they were not sitting still out of laziness. They were busy carrying everyone else. So the plan is the smallest amount of the right work that gets the result, done with somebody in the room with you. Not a printout. Not an hour-long routine you will quietly abandon by Thursday.
No. The whole point is a plan with an endpoint. The last phase, Prevent, is about handing it back to you: a short routine you will actually keep, the early signs worth watching for, and exactly what to do if it starts talking again. You should be able to leave here and stay gone.
